Hi Kelsey,
Are you possibly using some third party form layout tool/references in you app?
In our old forums I recall this same kind of issue, that 3rd party plug-in did not format the form to standards so CR doesn't know how to deal with it.
As Ludek suggested, remove the form change and see it it works again.
